Please note that the Blake Maternity Centre no longer take pregnancy referrals over the telephone.

If you are pregnant you will need to fill in a self – referral form which is available on the Portsmouth trust website ( see below ) .   Once your form has been submitted you will receive an email response giving you information regarding screening and having a healthy pregnancy.
