If you would like to register for our online services please call into the surgery and this can be arranged for you. Please visit reception with photographic ID and evidence of your address.
Did you know that you can also view your medical records?
If you would like access to your medical record please ensure you read the following leaflet - if you are happy to proceed then complete the 'Access Request' form below and bring to the surgery with a form of photo ID:

You can only request access to medical records once you have been registered with the practice for 3 months. Please note that if you request access to your medical records and this is approved by the GP you will only be able to view documents from the date that you registered with this surgery.

Please note that any digital image submitted to us will be stored within your clinical notes.
Please note that the eConsult service is only available during normal surgery hours which are Monday-Friday 8.00am – 6.30pm. If you require help outside of these hours please contact 111.
If you have a medical emergency please call 999. This is when someone is seriously ill or injured and their life is at risk.
Please also remember that pharmacies are able to help with minor illnesses such as aches & pains, sore throat, coughs, colds, flu, earache, cystitis, skin rashes, teething, red eye.
